The red light is flashing on your phone. The receptionist has just announced to you that Dr. Jones is on line one. You instinctively look at your watch, then the date. He has received his month-end reports. It is lunch time, his office is closed to patients, and he has two hours before his next patient arrives. Your mind goes blank, then a cold fog begins to creep over you.
