EHR Issues should review the various classes of clinical information (for example, prescriptions, allergies, specific information, and notes) and placement in the new eHR. as importantly, the process should verify that the converted data works in the new eHR. For example, the decision support rules or health maintenance items should be triggered by the converted information, and documents should print from the new eHR with THe JOuRNaL OF THe HeaLTHCaRe BILLINg aND maNagemeNT assOCIaTION 45 converted information. • The final conversion should be carefully checked to ensure that the converted information is properly placed in the eHR. In the vast majority of cases, the final conversion will occur after the new eHR is in use. Therefore, the conversion verification process should check the presentation of converted information with patient information that was entered into the eHR during the conversion process. STEP THREE: USE OF OLD EHR DATA IN THE NEW EHR – Old eHR data may be affected by a variety of issues, including the structural differences between eHRs, converted data presentation, and manipulation of the old eHR data into the new eHR structure. such manipulations could be quite basic, such as format or presentation changes. However, dramatic changes could include moving information from the old eHR face page to an image that is now buried in a list of old eHR information, or even recoding information from the old eHR codes to the new eHR coding system. Clinical and administrative users should both be trained on how the old eHR information is presented and accessed in the new eHR. users must be able to make any required corrections to the patient information. For example, the doctor may have to enter a patient’s old surgeries that were converted as a note from the old eHR into the surgery list in the new eHR. Conversion of old eHR information is necessary to support continuity of care and to preserve the patient’s medical record. Failure to adequately analyze and design eHR conversions could inhibit patient care and undermine the efficacy of your clients’ patient medical records. effective design of the conversion effort will guide vendors as well as create a standard to evaluate the conversion. That same process will guide your client doctors and users in transitioning their practices and their data to new eHRs.
